Dr. Anthony J. Diaz, P.T., D.P.T., Cert. MDT, FAAOMPT
Dr. Diaz was born and raised in Medina, NY, where he graduated from Medina High School in 1993. After receiving Master’s and Doctorate degrees in physical therapy he and his wife opened up Park Avenue Physical Therapy in 2001. Since then Dr. Diaz has completed multiple advanced training courses and continuing education courses with emphasis on manual therapy techniques, orthopedic and sports injuries, and ergonomics. Most recently he has become credentialed in McKenzie Diagnosis and Treatment.
The McKenzie Method is a biopsychosocial system of musculoskeletal care emphasizing patient empowerment and self-treatment. This system of diagnosis and patient management applies to acute, subacute and chronic conditions of the spine and extremities. It offers a reliable and practical approach that focuses on “what patients need” and not on “what therapists want to do“.

In 2020, he completed an additional Master’s degree in Orthopedic Manual Therapy (OMPT) and obtained fellowship status in manual orthopedic physical therapy. The American Academy of Orthopaedic Manual Physical Therapists (AAOMPT) is a national organization committed to excellence in Orthopaedic Manual Physical Therapy practice, education and research. Fellows of the AAOMPT strive to provide the highest level of musculoskeletal care through advanced manual therapy practice.
In his spare time, he enjoys spending time with his wife and family, running, weight training, and golfing.
Dr. Nadia Nava Diaz, D.P.T
Dr. Nadia Nava Diaz was born and raised in Buffalo, NY, where she graduated from Nardin Academy in 1994. After receiving her Master’s and Doctorate Degrees in Physical Therapy from D’Youville and Daemen College in 1999, she has pursued advanced training in Manual Therapy including myofascial release, soft tissue mobilization and joint mobilization. She has also advanced her education through multiple courses for spinal conditions using the McKenzie Mechanical Diagnosis and Treatment approach, shoulder and knee orthopedic and sports injuries, and various post surgical conditions and joint replacement. In her spare time, she enjoys spending time with her family, golfing, playing volleyball and cheering on our local Buffalo Sports teams.
Dr. Peyton Notebaert, DPT
Dr. Peyton Notebaert has resided in Western NY for 7 years after relocating from central NY where he graduated from Sodus Central High School in 2016. After receiving his Doctorate Degree in Physical Therapy from Daemen College in 2021, he has pursued advanced training in spinal and vestibular conditions. In his spare time, he enjoys spending time with his family, hiking, and traveling. He is an avid Buffalo sports fan.
Makayla Heideman, P.T.A is a resident of Medina where she graduated high school in 2021. Makayla is a 2024 graduate from GCC’s Physical Therapist Assistant program and enjoys helping people who are in need. In her spare time she enjoys reading, completing crafts and spending time with friends and family.
Jillian Cookinham, PTA
Jillian grew up in Albion and is a 2018 graduate of Genesee Community College earning an Associate’s Degree as a Physical Therapist Assistant. She enjoys working with patients to achieve their goals and the life-long learning opportunities available in the field of physical therapy. In her spare time Jillian enjoys spending time with her family, kayaking, crafting, and taking her dog on long walks.
